The ideal candidate has experience with treating addiction, HIV, and Hepatitis C, as well as comfort in treating chronic disease, mental illness, and those who suffer from homelessness. In this position, you will increase your knowledge of addiction treatment across the continuum of care, gain experience in integrated care beyond clinic to include substance use disorder and mental illness treatment. Provides preventive oriented ambulatory primary care as part of an integrated healthcare team in a Patient-Centered Health Home delivery model
Provides primary care to those with chronic disease, addiction, mental illness
Participates in the clinic performance improvement program, which may include monthly QI team activities, peer review, etc.
Works effectively in an integrated health care team including active scheduled management, team huddles, chart preparation, working collaboratively with team members, etc.
Provides support to and work closely with behavioral health for consultation support
Participates in weekly Integration meetings
M.D. or D. O. degree from an accredited United States medical school
Successful completion of internship and residency in primary care specialty with experience managing patients with substance use disorder/addiction
Proficiency in a language in addition to English, specifically Spanish
Board Certified in family medicine or internal medicine
Valid and active California medical license, without any limitations on license to practice in specialty
Eligible to participate in the CMS Medicaid/Medicare reimbursement programs
Valid and active DEA license with X waiver to prescribe buprenorphine; it does not have X waiver must obtain within 6 months of hire
Current BLS/ACLS certification or recertification within 2 months of hire

Like HAFC, Walden House has always served people who are uninsured, homeless and socio-economically disenfranchised, including those with HIV/AIDS.Haight Ashbury Free Clinics and Walden House have both grown over the years, becoming national models for community healthcare, substance use disorder treatment and mental health services.
