OurÂ busy Intake DepartmentÂ is seeking an Assessment and Referral Counselor who will beÂ responsible for conducting assessments of incoming patients and responding to requests for services and information. Making, documenting and processingÂ inquiry calls in accordance with Intake policies,
Conducting assessments on individuals experiencing crises,
Participating in screening patients for admission into both inpatient and outpatient hospital programs, utilizing pre-approved admissions criteria set forth by the Medical Staff,
Providing assessments of incoming patientâ€™s mental health/chemical dependency; including precipitating events, presenting problems, treatment/substance use history, mental status and risk assessment,
Making a determination of level of care by an evaluation of the patientâ€™s needs, advocating the lowest level of care consistent with those needs,
Completing admission paperwork,
Obtaining pre-certificationÂ in accordance with insurance requirements,
Completing and documenting In house assessmentsÂ with treatment or referral disposition recommendations,
Maintaining good communication with the utilization review, admitting, financial offices and other departmental staff.
