The Licensed Practical Nurse at the VA Puget Sound Health Care System's Silverdale Clinic, is responsible for providing direct and/or indirect patient care to an assigned group of patients. She/he will have the knowledge, skill and experience to carry out assignments involving, lab services-blood draw, point of care testing, treatments, diagnostic studies, and patient/family education. A recruitment/relocation incentive may be authorized. The LPN demonstrates active collaboration and communication with Patient Aligned Care Teamlet (Patient, Primary Care Provider, RN Care Manager and Administrative Associate) and Larger Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient). LPN Duties include but are not limited to: Provides nursing expertise in preventative screening, managing clinic flow, and data collection. Effectively utilizes PACT huddle tools/resources in pre-visit scrubbing process. Actively participates in daily huddle and team meetings. Provide virtual and telephonic care for patients via various platforms Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Accept delegation within scope of practice for tasks related to care coordination, procedures, and treatments. Delivers care to patients in a respectful non-judgmental manner and according to Patient Care Plan Demonstrates understanding of age - and population - specific characteristics that affect the health care needs of the veteran. Performs aspects of admission /discharge procedures as directed Collects patient data including observations of patient status. Accurately reports patient/client information. Recognizes subtle and significant changes in the patient in response to nursing care. Promptly recognizes urgent or emergent patient care situations and effectively intervenes; will seek RN/provider assistance for those requiring higher-level intervention. Communicates changes in patient condition/response to nursing care. Organizes/ prioritizes routine and non-routine nursing care activities Provides care to patients/clients appropriate to their cognitive, physical, emotional and chronological maturation. Accurately documents and retrieves patient/clinical information using electronic medical record. Provides effective education to patients and/or family members in relation to common disease processes, medications, and/or prescribed diagnostic and/or treatment regimes, as directed. Adheres to standards of practice and safe technique in preparation and administration of medicine, documents administration in BCMA. Observes patient for adverse effects from medication and reports to the team RN. Preferred Experience 1-2 years of PACT experience or Ambulatory Care Experience Community Based Outpatient Care or Rural Health Experience Tour of Duty: Various Schedules available.
