This position is located in the Primary Care unit at the Nashville VA in Nashville, TN. May need to travel to Outpatient Clinics. RN Pay Scale NG 57 A general description is: The PACT Coordinator is a Registered Nurse who serves as facility champion and coordinates local PACT implementation, training, communication, outreach and sustainability of the PACT model. The Coordinator plans, organizes, directs and evaluates the PACT program and reports related actions/activities to facility, VISN and national VA leadership. The incumbent serves as the PACT Program Planner and Site Coordinator. These functions involve active promotion of the PACT model among clinical and administrative staff within the Ambulatory Care Setting, educating an array of stakeholders; identify/addressing communication workflow challenges, facilitating meetings among teams, troubleshooting the program, and management of PACT related data. Areas of emphasis the PACT Coordinator will serve include but not limited to: Nurse orientation to PACT, EQM metabolic report card construction, FIT test monitoring, encounters not closed monitoring, notes not signed monitoring, PMDP due for controlled substance monitoring, EPRP action planning and monitoring, and clinical reminder completion monitoring. Will serve as a PACT RN on a PACT team in times where staff coverage is needed. Travel to CBOCs, at times, may be required. Position will report to a Primary Care Nurse Manager. The RN's role can include, but is not limited to the following duties: Establishing nursing diagnoses. Providing health education and procedural teaching to patients and significant others. Administering and evaluating pharmacological and other therapeutic treatment regimens. Administering medicine Establishing priorities and making ethically-sound decisions to ensure safe patient care. Assisting the physicians during diagnostic and therapeutic procedures to promote optimal patient outcomes by team collaboration. Responding to emergency situations to promote optimal patient outcomes by recognizing changes in patients' health status. Performing diagnostic studies within the scope of nursing practice as ordered by the physician. Documenting patient data to ensure continuity in the provision and coordination of patient care. Collaborating with other health care professionals. Communicating with veterans ad family members regarding veteran's care. May be responsible for charge nurse duties Managing follow-up care. Acting as resource to others. Participating in continuing educational activities, achieving/maintaining certification.
